New Hire Info For Indepentant Contractors (1099)

Welcome to the ProSource family!

Please read through this list of important documents and return any signed form or items as requested.

• Prosource Policies - Read and sign last page.
• Time Sheet Policy - Read and sign.
• Time Sheets - For your information.*
• Pay Schedule, Bi-Weekly - Read and sign.*
• W9 Form - Fill out form and sign.  You will need a company name and an EIN#.  Your DBA can not be the same as your name.
• Workers Compensation
  • Workers Comp Agreement - Everyone should fill this out.
  • Certificate of Insurance - Please provide this if you are already covered.
  • Workers Comp Acknowledgement - Fill this out if you are not already covered.
• On-line Payment Form - Fill out the form, sign and provide a voided blank check.

When completed, please bring your documents to the ProSource office (see Contacts for a map) or fax them to 713.272.8892
